There are many. But the most basic would be

1. Not having enough pipeline to work on

2. Not proactive

3. Not networking with the right kind of people

The major reasons could be the lack of proper planning, non-availability of adequate and desired resources, lack of continuous monitoring and non-reporting the progress and above all lack of appropriate recovery plans should the targets/goals slip away. Sometimes unrealistic targets/goals are also a reason for not achieving the same.

There could be multiple reasons why people do not achieve the set goals.
a.
Most Goals / KRA's and KPIs fail if the incumbent is not clear about the goals b.
Ensure that the Goals are SMART (Specific, Measurable, Achievable, Realistic, in a specific Time) c.
Motivation Issues d.
Performance Issues e.
Disengagement f.
Relationship with Boss g.
No proper handover There could be more instances, but these are few for your info.
