In operating in a high risk environment, a company needs to make sure they avoid risks especially on people, assets, and environment, in addition to other factors chosen to be important to certain companies (reputation for example).

In order to do so, there is a need to have a standard way of performing tasks, or at least planned work steps, which are summarised in method statement for construction, or task card for maintenance jobs, along with a proper risk assessment, in which determines operational hazards, the probability of happening, and lastly, how to control. Sometimes this is evaluated by cost versus realised benefit, safety versus operation affectivity (I guess some won't agree with the last one, but the reality in corporate world that almost everything is measured in monetary terms).

I believe6 sigma will do. The area of uncertainty behond6 sigma is0.2%. This means you control99.998% of each process. Suppose you have10.000 hours of work you will only stop operations for2 hours. But this is simply math and it's completly out of touch with reality.

It's not possible to mitigate risks to this level in operations, so the approach needs the element of slack, either in the form manpower, resources, insurance coverage, etc..

One cannot predict the future but it's possible to control the income events with the right method/approach.
