What are the requirements good detailing of reinforcement?

Good detailing of reinforcement is very important to both Designer and Contractor's so that during construction every things go well as design. So, In my opinion following things should be considered,

i) During Design select the proper size of bars and its spacing.

ii) Properly mentioned the diameter, spacing and any other extra length required for bars.

iii) Make detail drawings for critical areas like joints and connections.

iv) Finally don't use long words, use symbollic representation on drawings.

v) Follow some code or standard practice.
