Failure is inevitable, to deal with it is another challange one should not get bogged down by failure, infact one should investigate the causes and reasons and what went wrong and why. This will help the leader to mend his ways and fine tune execution plan.

Every place , everywhere and every one is applicable to fail more than one time , failure does not mean incapable , reasons are too many so we stop counting and better do actions, some time it requires a small meeting some times big one , some time on your own , the first important thing is to put down goal suppose to achieve, the required actions and responsibility and the outcomes ( which is failure this time ) . Then we start to go through them and mark the missed/not proper action taken. After we figure our the responsible sides we first find a solution to make up. After finishing this step we need to go directly and act for those who made this happen what is required is required , a training must follow to ensure they understand their job and what requires from them , then the last step is to put a new Standard operation procedure to prevent this from happening  .

  • To contact all other activities that may be affected by this problem
  • Check products to requirements and quarantine any nonconforming product
  • Record and supply all serial numbers, batch codes etc. to identify all parts, equipment or processes checked
  • Check any similar parts or equipment that have been generated from the same process, list details
  • Implement temporary fix to protect all other activities & stop shipping non-conforming products
  • Declaration:  Has problem been contained?  

 

1.      Defining the problem(failure)

2.      Select a team

3.      Brainstorming

4.      Fishbone analysis

5.      Identify key issues

6.      Ask Five times WHY? For each issue, get the root cause.

7.      Apply the corrective action upon root cause.

8.      Read corrective action to all affected areas

9.      Capture lessons learned.

10.  Monitor the results.

As a team leader has to  put the probability of the existence of the failure of the plan in place,because of my point of view we must make mistakes in order to learn or even to learn from others mistakes and with the possibility of the existence of this failure(expected) I'm going to study the consequences beforehand so that I avoid losses because it is not a defect in failure but the flaw in the persistent failure without a try in promoting to gain the best possible results
