significant changes in the structure of the organisation,

change of the premises,

following an enforcement action,

changes in the legislation,

risk assessments or accident investigations shows that the policy is no longer effective ,

after the introduction of new or changed work methods or processes

Besides periodical reviews or indications of a need for improvement such as an accident, any change in and around the organization should stimulate that organization to review their health & safety policy.

There might be changes in the organization or physical changes in the workplace (expansion, change in working location with different influences), growth in number of employees, new information about risks involved, but also changes in the surrounding of the organization can be a reason to review the policies used. For instance, a new office building next door might increase the liability and force a company to consider changes.

Many top companies adopted the Management of Change. It is a system where all employees can suggest to make changes in company policies and not only HSE. If an employee thinks that a certain policy is outdated or incorrect then the employee needs only to follow certain guidelines on how to prepare and submit his suggestion to the committee. Usually an employee is awarded for his contribution.
