The possible probabilities of Kaizen failure could be:

1. lack of understanding between the team members

2. expectation of quick outcomes

3. lack of management commitment

4. improper utilization of 5S 5 WHY 

Mainly the Continues improvement leader faces lots of difficulties and when it comes to team building the ideas of each team members. because every individual has a different opinion and when it comes to a team the solution should be one so getting the ideas and combining it with a one solution part will be a bigger task
