(2) Are the results obtained from the monitoring and measurement activities reported accurately (3) were the results reported on time to the appropriate personnel or office? (4) Are the results obtained from the monitoring and measurement activities fall in the allowable ranges? (5) if no to (4), what are the next action and have they been taken
