Usualy processes come in to the system after understanding the operations and running them successfuly. The process is nothing but a way that's found more optimal way of doing a job. This can be derived from best practices of industry, individual experiences or through finding the right way of doing after multiple iterations.

Hence, processes come from  collective wisdom, collective experiences. I feel, once the process is in place, we always can improvise on it if we find it's not optimized. So, it's Process over people that matters for successful project management.  A good process can make a mediocre person work well.
