If your project is going wrong, what are you doing to rectify this scenario urgently?

stop the project and review my steps and detect the problem and finding a solution to these problems

you need to find out the reasons,  make a remediation plan and then action with constant monitoring

it is a genaric q? and  i believe the answer is well known. 

but i believe the first thing you should do after resolving the scenario is to question your project management skills :)

Investigate and find where the problem is and what is the intensity of problem.  Make a proper plan and rectify it accordingly.

Risk Management can play a better role here, as you first have to identify what are the risks that driving the project in the wrong way, once foundout analyse those risks and then address all those risks in the most proper way and then follow those risks lest those should not arise again.  

Stop the project. make an independent review of the project and identify the key easons/issues. Analyze these problems/issues and make a plan to rectify them. If necessary, change the project methodology.

To start with:

STOP whatever is being done

IDENTIFY what is going wrong

DRAFT a solution - you must think up a plan B

INFORM everyone concerned i.e. development team, client

GET-BACK-TO-WORK self explanatory

MAKE sure it doesn't happen again

First of all I will call an urgent meeting of all concerned department , need to know from where this project is diverting, assess the loss caused and look means to resolve the cause my rectifying defect, since abandaning the project at the final stage may cause a massive lossnot only financially but also dent reputation in long ri.r

This will be subject to size and complexity of the project in hand;  

a/ Check if project requirements are fully complete and agreed by all

b/ Check for scope creep, ring fence what is in and what is out and deal with

c/ Check if a structured project management methodology is being used and adhered to

d/ Check if change control process is in place? If not this should set up as a priority

 e/ Check if project objectives are sufficiently clear to all concerned. 

I do my best to stop project from continue in wrong way.If I succeed I will orient my team to prepare optimised solutions to rectify my project and make corrective plan to my project to reach our goals from the project.
