If a team fails who will be responsible? Team leader or Management during team selection and why?

Team leader, will be more responsible, not the management.

Team leader will be more responsible because he is also the performing as a facilitator between Management and other teammates. So, Team leader might not telling or providing the exact role to his team members that could be the reason they are not performing according to the standards which are set by the management.
