In which instances have you experienced logistics challenges arising from poor communication?

I had to coordinate a logistics project involving a factory in India, a customer in South America, and multiple offices in India and the US. Since business functions were handled seperately across each of the offices, communication was a key factor in the smooth flow of goods. Any delay added minimum one week to the transit time as the feeder vessel would miss the connection to South America.
