What is meant by lesson learned data base?

AT the end of each project or eahc phase of the project , you do a root cause analysis session with all project team .

You identify what were the pain points and how did you solve them or what was supposed to be the correct solution .

then you start to document all this in a database for future use . this is called lesson learned database
