There are many causes of belt failure :

Like pulley misalignment will cause the belt to rub. This rubbing will generate heat that the belt is not designed to deal with, causing weakening and then failure.

Too much tension and the belt will stretch beyond tolerance.Not enough tension, and the belt will snap tight each time the engine is started, causing weakening and then failure.
