I have to present monthly greviance recorded report. I mean Greivances recorded, follow ups, solved cases, etc

If anyone can provide the result of any operation in the report that can be the best but we cannot measure the grievance solving result in any unit. But as per my best knowledge and format I used following can be included in the report
1. Process detail and stages
2. Name of the counsellor team and there detail
3. Table having detail of Date, Name , Designation, Department and there feedback ( it will effective if we ask them to rate between1-10)
4. Suggestion of CAPA (corrective action and preventive action plan) by panel member against every problem diagnose during process.
It could be other thing can also come under reports depended to industries and program. If you can provide me detail I can try to give form a sample report for you
