The most likely diagnosis?

A-year-old white man recently experienced bilateral parotid swelling and weight loss. His serum creatinine rose from1.2 mg/dL (eGFR, mL/min) to3.1 mg/dL (eGFR, mL/min) within three months. He has been taking NSAID. He was found to have bilateral hilar lymphadenopathy. Serum calcium level was slightly elevated at.8 mg/dL. Urine protein/creatinine ratio was0.3. Urinalysis showed only1-2 RBCs/HPF and1+ protein. Other serologies, including ANA, ANCA, hepatitis B and C, are negative. Serum complement levels are normal.

Simple and straight forward case of  sarcoidosis and as srum Ca level s little high so some risk of stone formation also

it looks like sarcoidosis , which is not a very common disease. i will ask for angiotensin converting enzyme  levels in blood.

It could be sarcoidosis but there is not enough data. How old is he? Why he used NSAID (it is another possible reason for renal failure here).
