How can we write inclusive and informative reports about our work progress?

For me as a civil engineer . when i have been chosen as a site engineer for any consruction project , I will raise the daily , weekly and monthly reports to my bosses telling them about any thing occured during time of project performance and i promote my reports with photoes , my reports contains :

1- No. and type of foremen , workers , equipments and technicians whom working for the project

2- Ratios of performance till the end of daily / monthly  works

3- Productivity of all workers and technicians and equipments

4- Safety , Risks reports

5- Any problems may occured during the work and their solusions
