As a teacher, How do you keep parents aware of their son’s or daughter’s grades?

If there were no problems then during a "catch up" in parent's evenings at the school, this would be in the form of a general discussion on their child's progress and future plans.  If there were a problem that I felt needed addressing I would contact them to arrange a meeting whereby we could discuss the problem and work out what remedial action to take.
