Thanks for inviting me to answer this question.

I think it is lack of communication.

This is because in my opinion, due to the lack of exact clarity in vision and the ability to communicate it to the subordinates (sometimes multinational or same nationals), a lot of misunderstanding arises between everyone involved.

If the communication is effective between a manager and his subordinates, then a lot of problems can be solved, as the manager is able to recognise whose potential is more than the others and put them in suitable possible roles to achieve his tasks as a manager.  It will also encourage individuals to keep their egos aside (including the manager himself) and work as a team.

Today, due to jealously between the earnings of individuals (people who deserve it,  do not get it and people who don't, gets huge sum - is itself unfair), no one is willing to communicate with each other.  Due to this reason, it has become an upheaval tasks for the manager, especially when he is facing unpredictable situation of risks and the target he is expected to achieve within a strict period.

The manager is not able to find suitable solutions since he may be incompetent, in comparison to his suboordinates.  Maybe, he has all the information,  but does not know how to connect it and he is not willing to share his knowledge with his subordinates, who may be able to find it for him.

Therefore, I think if a manager is wise, he will be able to communicate with his team (which should not be a challenge) to achieve the desired results.
