A defect is a flaw in the completeness of an intended product which while it may still function or perform the intended purpose, there will be a reduced effectiveness or constant hitches due to the flaw. A failure is a complete lack of realization or achievement of product quality or form thus rendering it useless in performing the intended function.

If any Mistake is done while coding its a failure, but the same is found by the tester then it will be the defect which will be accepted by the developer.

When a defect reaches the end customer it is called a failure and if the defect is detected internally and resolved it's called a defect.
