Pulmonary surfactant is a complex mixture of phospholipids and proteins that serves to reduce alveolar surface tension. It is formed by type II pneumocytes from about 20 weeks of gestation. Surfactant creates a continuously reforming surface layer over the alveoli which reduces surface tension, prevents atelectasis and maintains alveolar stability. A deficiency or dysfunction in pulmonary surfactant production causes Respiratory Distress Syndrome (RDS). Risk factors associated with RDS are shown in Table 1, and incidence of RDS at RPA 1992-8 in Figure 1. Exogenous surfactant has been shown to reduce neonatal mortality, death or bronchopulmonary dysplasia (BPD) and airleaks

 All Neonates 29 to 32 weeks( Rescue treatment) - If need intubation for respiratory distress - do it- BEST - But mortality with RD not changed if no surfactant deficiency 

surfactant is  indicatedin premature  babies where their lungs are not completely matured enough..administering surfactant improve the maturity of the lungs ..usually it is administered within 24hrs after birth foe better results

this is use for new born that has problem with the respiratory system such as secnodary surfactant defieciency. If not administered it could lead to morbidity. while in the womb fetus only gets oxygenated blood form their mothers, if surfactant from the lungs is lesser than normal or not present at all,babies will have problem breathing.
