Too much confidence.

Unrealistic expectations.

Improper anticipations or calculations.

Inadquate resources to facilitate the decisions.

Conflict of interests.

Egoism & selfishness.

it can fail for a number of reasons but the most important reason being not doing a thorough plan before taking a decision or if its a team role, not taking the team in confidence.

Decisions fail for the following reasons:

- Lack of useful information for decision making

- Unreliable informations

- Non-structured information

- Very limited choice of solutions

- Irrelevant information

- Do not decipher the situation

- Do not detect the problems

- Poor analysis

- Do not know exactly what we are searching

- No modeling of the problem

- Non-use of the model

- The precipitation

- The inappropriate choice of decisoins, unsatisfactory

- Non-founded intuition

- No tests or trials insufisants

- No turning back to put back the problem
