Two words: "document it".

 

Do not approach him/her verbally, send him/her an email with your idea/work and that is a solid proof that it is coming from you and not from anyone else ;)

 

Yes, I have done that many times and it always worked :D

Always give reports in written form along with your signatures on it and try to make them complex making it hard for the manger to present.

Never show all your cards. if your boss is habitual of getting credit over your assignments, then keep one or more factors yet to be explanined. when he would be called for explanation he will try to contact you this will make the top management reveal regarding the initiator
