There are several factors which may influence into such practices.

1. The most important is local law and degree of its implementation.

2. Cost of implementation

3. If safety system and protocols are not developed properly, they may causes unnecessary delays in the decision making process which often tends to infuriate field engineers and workers. 

4. Again, Is it possible for a worker to sue the company for safety related malpractices ?

5 Local environment and culture. In some cultures, following safety procedures is taken as a sign of weak personality, Although it is very subjective but unfortunately a reality.

6. working conditions. You cannot force a worker to wear heavy safety gear in soaring temperatures unless they are actually motivated and concern about their safety.
