Why is implementation stage failed in most projects?

implementation is an action stage were what has been plan at the formulation stage need to executed,.At this stage there is always conflict of interest between different parties,bureaucracy,diversion of funds,unneccessary delay which may arise to different issues like the management,management information system,research,finance,marketing etc effective and execution of any job can not be smooth,which can be part of the failure at the implementation stage.
