If bad decisions are made with regards to business and results in a loss, then sorry to state the intent does not matter much. Bad decision is a bad decsion regardless of intent in business.
Bad decision is bad decision NO any body asked good.
In business, we should be result oriented. Bad decisions may lead to disasters that may destroy the business, can we say the intentions were good. No sir, decision are judged upon results. Employees including managers are also evaluated upon results and achievements.
As we can never express bitterness as unpleasant sweetness, so same expression suits for this question; I would say, "BAD is BAD by all manners yet the treatment should be different with good intention and that bad decision should be taken as lesson learnt & area for improvement".
Bad decisions are Bad. It can never be Good. If it is Good then the decision must be also Good.

Bad decisions are always bad if with good intention or bad..
I feel really bad to say this but I don't the power to see intentions so it will come to results. If there was a good communication and everyone was involved in the decision then no one will take fully the blame but if you are the only one to take that decision then you have to assume your responsibility.
That's not only part of your position at the company but that's part of life in general.
Bad decisions, if you can adjust the other conditions so as to make it to end up with good, it doesn't matter. at the same time it should not have any impact for personal situations badly.
Good intention is supposed to be a prequisite quality of a Manager other wise it'll be discarded from the organization. So a bad decison with good intention has nothing with the business rules since it yields for a failure achivement and then remains bad
Finally I have to note that we can learn so luch from bad decisions
