Why is management and system blamed for poor quality management instead of employees?

Because it is the fault of the management.

If the management have hired the right people, trained them properly, given them the right equipment, set up the right system and supervised the whole thing correctly, then there are no problems.

If managment are competent then there are no difficulties with employees.
