How do we ensure to cover all the test scenarios?

To check whether all the scenarios is been covered we used to prepared Traceability Matrix which is checklist prepared on the basis of Business Requirement Documents.

Though test cases cannot cover % test coverage the business requirements Vs Test cases can identify the test coverage based on the application.
