Why is the problem of deliveries with wrong specifications increasing with overseas shipments?

Despite the buyers efforts to write detailed specifications for all the items being ordered from overseas procurements , still items with wrong specifications continue to be delivered to end users . could it be a problem of so many middle men in the supply chain today??
