For me it depends on the type of project it is and the environment it is run in. If it's a Manufacturing project I often use the Six Sigma methodology and follow the DMAIC concept because it is more data driven. However, if it's a new manufacturing footprint I would follow the PMBOK process of IPEMC. However, in both instances I have predefined process templates that I generated in my PM software. Many use MS Project I use Merlin/ITask both Mac PM software options that are great for tracking. Weekly meetings with the team clearing roadblocks in an open environment helps ensure the project stays on track. Also choosing the right people for the team, its good to understand tools like MBTI/Insights or Belbin to generate your project team and ensure that you have the right balance of talent to drive the project to fruition.
Being a technical architect who does project managing, Object oriented Design (OOD) and reusability are the first step in the start any of my projects management plan(s) with all its phases and millstones - including project's Calendar, Tracking and Assessment.
