I think the best way to fight falsified content in your Performance Review is to have a one-on-one discussion with your Line Manager and ask him / her to give practical and real-life examples of events which led them to include those comments in your Performance Review. It could either be a case of gross misinterpretation or a personal grudge against you. Asking for quoting real-life events connected to those comments will help you in both the situations i.e. either correct the misconception / misinterpretation in your LM's mind or expose his / her wrong and baseless comments.
